Licensing Program Details

There have been many questions surrounding licensure. Below are answers to some of the most commonly asked questions. If you have a question and do not see your answer here, please carefully read through the complete application form before completing it. Also, the TDSHS website has a section for Frequently Asked Questions.   1. Does my transcript have to be sealed, or official?
    Your transcript must be official. It cannot be a copy, but it does not have to be sealed.

  2. What if I lost my membership card? Can I send in a copy of my ALTA certificate instead?
    If you do not have your membership card, you may send in a copy of your ALTA certificate instead. If you have lost your certificate, please contact the ALTA Office to receive another. The cost is $10. Please note that these are both critical pieces of documentation that verify your membership. We recommend you keep a file specifically related to your ALTA membership documentation.

  3. What if I didn't put all my renewal stickers on my certificate?
    That is fine. The State has a list of all current ALTA members, so a copy of your certificate, with or without the stickers, will do.

  4. Why didn't I get an application in the mail?
    Applications are being mailed out in stages and they are only being mailed out to Texas residents. If you did not receive an application and would like to apply, please download one from the TDSHS site- http://www.dshs.state.tx.us/dyslexia/default.shtm.

  5. Can members outside the State of Texas apply?
    Yes. However, you will still only be licensed in the State of Texas.
